WordPress Icon Image

Can anyone tell me how to get to my web site icon on my WordPress page?

I followed the instructions and put this code: in the "head" in the "header.php" file. I also uploaded my image to the img folder in my theme. I tried changing the code for "img / favicon.ico", as the link, still no luck. Any idea what might be doing wrong? Thank you.
